I've had similar problems recently in trying to get images to look clean and sharp.

The way that I try is starting of painting using the airbrush and paintbrush. I then use a little dodge and burn to get shades and gradients and transitions to look at little better. Then use the smudge tools to try to refine the blurry bits and sharpen the image (this takes along time to do however).

Then touch up with the airbrush and paintbrush tools. Repeat the steps as necessary.
